TO: JARED JOHN HEILMAN, nonconsenting father/alleged father YOU ARE HEREBY SUMMONED to appear on the 28th day of August 28, 2020 20 at 1:30 p.m. at the Clark County Superior Court, 1200 Franklin Street, Vancouver, WA 98660 and defend the above-entitled action, and answer the Petition of the Petitioners, and serve a copy of your answer upon the undersigned attorneys for Petitioners at the office stated below. If you fail to do so, judgment may be rendered against you according to the request of the Petition for Adoption and the Petition for Termination of Parent-Child Relationship which has been filed with the Clerk of the said court. YOU ARE HEREBY NOTIFIED that a petition has been filed with the Clerk of the above court requesting that the parent-child relationship between you and the above-named children be terminated. The object of the action is to seek an order terminating the parent-child relationship between you and the children and a Decree of Adoption declaring the petitioner(s) to be the legal parent(s) of the children. The child, Logan Eli Heiman, was born on June 30, 2006 at 10:39 am in the city of Longview, County of Cowlitz, State of Washington. The child, Bode Carter Heilman, was born at 11 :00 am on August 11, 2009 in the city of Longview, County of Cowlitz, State of Washington. The children's mother is Melissa Heilman. You have been named as the father of the children. The Court hearing on the Petition for termination of Parent-Child Relationship is shall be on August 28, 2020 at 1:30 p.m. at the Clark County Superior Court located at 1200 Franklin Street, Vancouver, WA 98660. YOUR FAILURE TO APPEAR AT THIS HEARING MAY RESULT IN A DEFAULT ORDER PERMANENTLY TERMINATING ALL OF YOUR RIGHTS TO THE ABOVE NAMED CHILDREN. You may respond to this summons and notice by filing a written response with the Clerk of the Court and serving a copy of your response on Emily T. Roberts, attorney for petitioners, whose name and address appear at the end of this summons and notice. If you do not serve your written response within thirty (30) days after the date of first publication of this summons and notice, the court may enter an Order of Default against you permanently terminating all of your rights to the above-named children. The court may, without further notice to you, enter an order terminating your parent-child relationship and approving or providing for the adoption of the above named children. You are further notified any non-consenting parent has the right to be represented by an attorney, and if you are indigent and request an attorney, an attorney will be appointed for you. You are further notified that your failure to respond to this termination action within thirty (30) days of the first date of publication of this summons and notice will result in the termination of your parent-child relationship with respect to the children. You are further notified that you have a right to file a claim of paternity under Chapter 26.26 of the revised Code of Washington. You are further notified that your failure to file a claim of paternity under Chapter 26.26 of the Revised Code of Washington or to respond to the petition for termination of parent-child relationship which has been filed herein, within 30 days of the first publication of this summons and notice is grounds to terminate your parent-child relationship with respect to the children and such relief will be requested at the hearing above. YOU ARE FURTHER NOTIFIED THAT IF THE CHILD IS EITHER: (A) A MEMBER OF AN INDIAN TRIBE OR (B) ELIGIBLE FOR MEMBERSHIP IN AN INDIAN TRIBE AND THE BIOLOGICAL CHILD OF A MEMBER OF AN INDIAN TRIBE AND IF YOU ACKNOWLEDGE PATERNITY OF THE CHILD OR IF YOUR PATERNITY OF THE CHILD IS ESTABLISHED PRIOR TO THE TERMINATION OF YOUR PARENT-CHILD RELATIONSHIP, YOUR PARENTAL RIGHTS MAY NOT BE TERMINATED, UNLESS: a.
